The Hierarchic Dementia Scale was originally developed by Dr Martin Cole and Dr Dolly Dastoor in the 1970s to rate the level of cognitive functioning in people with dementia. It is not a diagnostic tool. It may contribute to care planning. It was revised in 2015 by the Dementia Training Study Centres who also developed this HDS-R App. You can record the scores obtained during a HDS-R assessment using this app, and email reports and a customizable care plan out of the app.

- To complete an assessment you will need a Hierarchic Dementia Scale-Revised (HDS-R) kit

- Reports and care plans can only be obtained by emailing (exporting) from the app. Once emailed (exported) they will be permanently deleted from the app and iPad

- Incomplete assessments will be saved within the app until marked ‘complete’

- Care plans can be customised once they have been exported from the app.

- You will need to have your email client set up on this iPad to export the reports and care plan

- To protect your clients’ personal information you will not be able to input your clients name in the app. Their data will only be identifiable by a unique ID. See disclaimer in the app for your responsibilities regarding this.
